Derealization

Definition

Derealization refers to the experience of the surrounding world feeling unreal, unfamiliar, or distorted. Patients may feel like they are in a dream, seeing through fog, or observing the world through glass.

Common Symptoms

  • Surroundings appear unreal, blurry, or flat
  • Distorted sense of time (too fast or too slow)
  • Sounds seem distant or unreal
  • Familiar places look strange
  • The world feels like a "set" or "painting"

Common Triggers

  • Severe stress or anxiety
  • Traumatic experiences
  • Sleep deprivation
  • Substance use or withdrawal
  • Other mental health conditions

Coping Strategies

  1. 5-4-3-2-1 grounding: Name 5 things you see, 4 you can touch, 3 you hear, 2 you smell, 1 you taste
  2. Cold stimulation: Wash face with cold water or hold ice
  3. Physical movement: Light physical activity helps return to the present
  4. Social connection: Talk with a trusted person
